Simply call us at 716-710-7022 or email us. We will help you schedule a time that fits into your schedule.
2. What should I expect at the first visit?
You will be meeting with a Certified Diabetes Educator who will review your medical history, provide education and work with you to set goals that will help improve your health. We encourage you to bring a list of your current medications, blood sugar readings and a 3 day food record.
4. What is a Certified Diabetes Educator?
This is a health professional that has been certified to provide education and self-management training for Diabetes and Pre-Diabetes. This professional has accrued over 2,000 hours of Diabetes education hours and has passed a nationally recognized test. Here at Personalize Health Nutrition we are both CDEs, so you can be reassured you are getting experience and quality guidance.
