Candi Possinger has been a Registered Dietitian for thirteen years and a Certified Diabetes Educator for 10+ years. Candi’s goals for Personalized Health are similar to those of her career – emphasizing patient satisfaction and helping to achieve quality outcomes for both patients and their healthcare providers. She has had extensive involvement in education, dedicating her career to both patients and students. Candi holds a BA in Math and a BS in Dietetics from Russell Sage College and participated in a dietetic internship through the University at Buffalo (through which she also received her Master’s in Nutrition).

 

Prior to forming Personalized Health Nutrition, Candi held the position of Manager of Nutritional Services at Catholic Medical Partners.  In this position, Candi had the privilege of creating and managing a nutrition program that embedded Registered Dietitians directly within medical practices throughout Western New York.  Candi has also worked in various positions in her field – most notably she was Program Coordinator for 2 local Diabetes Education programs and she was Clinical Coordinator for the University at Buffalo’s Dietetic Internship Program.   In addition, Candi is a Clinical Instructor for both D’Youville College and the University at Buffalo.

 

Candi’s leadership skills have enabled her to hold many positions on professional committees. Currently she is Treasurer for the Western New York Association of Diabetes Educator, a Past President of the Western New York Dietetic Association and sits on the New York State Board of Dietitians and Nutritionists.

 

Candi has also been published in both scientific journals as well as popular press, such as Diabetes Self-Management magazine on the topics of Nutrition and Diabetes. She has also appeared on several local radio and TV shows and has presented at various conferences.

Candi has been recognized with the New York State Recognized Young Dietitian of the Year award as well as being named a Rising Star from the WNY Cystic Fibrosis Foundation.

Lynda Hart, MS, RD, CDN, CDE

Lynda is a Registered Dietitian and Certified Diabetes Educator.  She received her Masters of Science in Nutrition from the University at Buffalo.  

 

Co-creating Personalized Health Nutrition has allowed Lynda to fufill her professional and personal goals.  Previously Lynda worked for Catholic Medical Partners where she worked one-on-one with patients to help them to achieve their own personal goals and is excited to be able to continue to do so for Personalized Health Nutrition.

 

Lynda has held the co-chair position for Professional Issues on the Western New York Dietetic Association board and was honored to be presented with the New Professional Award in 2014.  Lynda has presented her research on flaxseed intake at FNCE (Food and Nutrition Conference and Expo.  She has also worked for WIC (Women, Infants, and Children.

 

Lynda is originally from Michigan, but spent many years living in New York City where she found her passion for all things health related.  She now lives in Orchard Park with her husband and twin sons.  She spends her free time enjoying the outdoors of WNY with family and friends, traveling, as well as creating delicious food in the kitchen.

Carolyn is a Registered Dietitian. She holds a BS in Exercise Science and MS in Nutrition from the University at Buffalo, where she also completed her Dietetic Internship. 

 

While completing her Master’s degree, Carolyn worked with UB Sports Nutrition where she provided education, resources, and counseling to many of the UB Bulls Men’s & Women’s sports teams. In the UB Sports Nutrition Lab, she participated in conducting several research studies observing the effects of Vitamin D and Exercise. Carolyn also worked for several years at Catholic Medical Partners as an assistant to the Nutrition Program. There, she compiled data reports that outlined patient outcomes and helped determine how to optimize Registered Dietitian services for the program. 

 

As a member of Personalized Health Nutrition, Carolyn feels privileged to continue her professional career in a position that allows her to work one-on-one with her patients to help them succeed in reaching their health goals. She is adamant that nutritional counseling should be tailored to each individual, and is committed to combining evidence-based knowledge with goal-oriented strategies to achieve quality, sustainable patient outcomes. 

 

Carolyn grew up in Boston, NY and currently lives in North Buffalo. She stays active by taking long walks with her Bernese Mountain Dog, who motivates her to keep moving during our long Buffalo winters.  She loves to cook, and loves to talk food!
